The toxin in the thorn later caused Piper to fall into a coma. She then became stuck in the Cosmic Void with Cole. Even though the wound was eventually healed by Wyatt, Piper remained in a coma and was dying from a broken heart until Leo returned to her, having fallen from grace and becoming mortal.
