The title originated as the LOP 24/7 Hardcore Championship in the e-federation for the Psychotic Wrestling Alliance (PWA), the LOP Hardcore territory. The original thread saw the formation of the Madcore Wrestling Alliance, composed of Tromboner Man and RaTo, the two members who would gain fame from the MWA name, Crazy Ash Killa, Eric Scorpio, Homicide and The Rik. The thread became inactive, leaving the LOP Hardcore roster to head to the PWA. On January 8, 2011, the thread was restarted, with Jude Maxwell being the inaugural champion.

During the history of the championship, hundreds of people and non-living objects have gained the title. Non-living champions was a challenge for competitors. For example, when the The Rik's horse won the title, they had to find someway to win the title. Another champion Murray, a disembodied intelligent talking skull, had no body and no shoulders to pin, so competitors had to prop him on a bottle which could act as his body.
