Despite previous claims that id would not be modifying the versions of Doom or Doom II included with Doom 3: BFG Edition, the secret levels have been heavily censored; all Nazi imagery, including swastikas and portraits of Adolf Hitler, have been removed, and most disappointingly, all Wolfenstein SS enemies have been replaced with standard Former Humans. Additionally, because of this removal of the references to the original game, the level has been renamed "Keen", as the cameo appearance of that character has not been removed. This also seems to be the case in the so-called Doom: Classic Complete package on Steam. Also, on the BFG Edition, the music track "The Ultimate Challenge", has been switched out with the track "DOOM".

Al igual que "Wolfenstein", "Grosse" es una remake de un nivel del juego anterior de Id Software, Wolfenstein 3D, es decir, el nivel del mismo nombre desde el final del primer episodio de ese juego. También como "Wolfenstein", "Grosse" presenta el enemigo exclusivo del nivel secreto Wolfenstein SS, aunque el jefe que aparece en el mapa original, Hans Grosse, ha sido reemplazado con un Cyberdemon.
