That's a good question, and I don't think that has been revealed yet. It makes sense for Urahara to have implanted it in her when he gave her that gigai after she lost her powers. However, according to Aizen, the Hōgyoku was the reason she lost her powers in the first place. So it's not clear when exactly Rukia was implanted with it.
