Heart Leeches were native creatures that live in the Hollows. They are not part of the Locust Horde, and their existence was known to the human race during the Pendulum Wars. Heart Leeches kill by embedding themselves into their victim's chest and slowly tear open the rib-cage. The heart is then devoured inside the chest cavity as the leech then exits the deceased victim, which is why they are called heart leeches.
