That's about 261 petajoules of energy in a normal 1 kg matter/antimatter reaction. 4HE High-Fusion Reactors were not easy to retrofit, even by the 27th century the complex engine design was incapable of being sporadically integrated into a ship. Thus making any new ship or ship class original installed with the design. The only known ship wide disaster induced by a 4HE High-Fusion Reactor was in the early 26th century when a starship miscalculated the intermix ratio and caused to severe of a helium-4/antihelium-4 reaction.
