The Bow of Pestilence is one of the weapons of the Four Horsemen. It is used by Pestilence. The Bow of Pestilence is obtained after completing the quest The First Horseman - Pestilence. It's a unique weapon because it deals disease damage along with regular damage. This makes it a deadly weapon because whenever the enemy is damaged by disease, the opponent's skills are reduced. Recovery takes one minute per level reduced, which can potentially be a long time. Because of this, the only way to avoid the disease damage is to use Protect from Missiles or Deflect Missiles so as to not take damage from the disease. This is because you will not take damage if the arrows do not hit.

This is because you will not take damage if the arrows do not hit. Pestilence arrows, which can be bought from Reldo for 1k each after the quest increase the disease damage's max hit from 170 per hit to 280 per hit. The regular damage has a max hit of 300 per hit. The bow has a special attack where it will fires 3 arrows in rapid succession at a single enemy, or 2 arrows each at up to 4 enemies, with each arrow having a disease max hit of 350 per hitsplat and a normal damage max hit of 640, and the arrows will never miss. The max hit is increased to 780 if using Pestilence arrows. The special attack is triggered randomly, about 3% of all attacks will be a special attack.
