Because of its massive chakra requirements, Makoto is left immobile during and after the technique, therefore he only uses it as an absolute last resort, lest he use it too early in a fight and be left without chakra and vulnerable. He has only used it once, and it was on behalf of the Fifth Mizukage. She was attacked by a group of about a dozen former Oto-nin, and in an attempt to prove himself trustworthy, he used the Aqua Susano'o to defeat the attackers singlehandedly.
