When the test subjects opens a door a scythe will swing down and impale it's victim if the player doesn't push the right buttons fast enough. It works similar to the Shotgun door traps which appeared in Saw: The Video Game and reappear in the sequel.

If the player chooses the path of blood in the end Campbell then meets Jigsaw personally inside a door and Jigsaw tells him that his son is outside the building. Campbell gets very angry and tries to assault Jigsaw but he is impaled by a swinging scythe.
