O'Reilly was one of the Marines sent aboard the USS Sulaco to investigate the ship. O'Reilly was one of the several Marines injured in the internal explosion aboard the Sulaco that occurred just as Cpl. Winter was attempting to board the ship via the umbilical from the USS Sephora. O'Reilly was being helped by Private O'Neal and Paluski when Cpl. Winter boarded the Sulaco. O'Reilly was among the several Marines who were presumably devoured by Xenomorphs as he could not be found after Cpl. Winter and Sgt. Keyes returned to the hangar with the flight recorder.

- O'Reilly is the man in charge of the raider camp in Freeport in 2197.
- Drill Sergeant O'Reilly was an Orbital Drop Shock Trooper and instructor at ODST boot camp in 2525. On the second day of training O'Reilly would walk up and down the line, shooting the recruits in the leg with a pistol loaded with Tactical Training Rounds and telling them to run. Those who failed to respond would be shot again. Another major exercise was to place the recruits at the base of a mountain and tell them that if they could neutralize the instructors and make it to the top, they would eat and sleep in their barracks that night. Failure would mean a cold and hungry night on the mountain.
- O'Reilly was one of Colonel Stuart's mercenaries who was involved in the takeover of Dulles International Airport to free General Ramon Esperanza.
- O'Reilly was an Earthforce Lieutenant General at the time President Morgan Clark declared martial law. After Clark dissolved the Earth Senate and as the Elite Guard assaulted the Senate building, O'Reilly delivered General Hague's last message to John Sheridan: "Everything's gone to hell, John. You're on your own."
- Dr. O'Reilly was one of the ten applicants to be assigned to wash House's car. Although the other nine started working on the task, O'Reilly was unable to help due to being confined to his wheelchair. He started complaining that he didn't go to medical school to do this type of work. Amber Volakis started complaining too, saying House had already decided to cut them and just wanted to get his car washed as a bonus. She walked out and took O'Reilly and seven of the others along with her, leaving only Jeffrey Cole to work on the car.
