Runty was a member of a family of T-virus survivors. He dressed normal, looking like a sports fan and knocked Alice out before throwing her into the dog cage. His death by the escaped dogs isn't seen but he can briefly be seen running but one dog only seconds away from tackling him in the air.

However, it is revealed that Runty is in fact Billy's Imaginary Friend—giving explanation as to why he was not under Fuse's control. When the player infiltrates Fuse's lair in an attempt to weaken its defense shields, Billy shows up with Runty; offering assistance and Runty is sacrificed—giving all of his Imaginary Energy to lower the shields to Fuse's Labyrinth.
- Runty is the offspring of Beauty. As his name implies, he is the runt of the litter of four pups. He can be found in his own little area behind and to the right of Beauty, frolicking near the lava pools within Beauty's Lair[[:File:|[, ]]] of Blackrock Caverns. Further evidence of his runt status lies in his buff:
* File:Ability hunter pet corehound.png [Almost Ferocious]ω ϖ—He's trying his best... Players wishing to kill his mother should avoid killing him prior to engaging her, as she goes berserk and is able to quickly wipe groups.
