Medusans are known throughout the Federation as mathematical and navigational experts without equal. Their thoughts are the most sublime in the galaxy, however, their physical appearance so utterly hideous that the sight of a Medusan would render a human mad. Vulcans are capable of viewing Medusans, but only with the use of a specially filtered visor. For these reasons, Medusans usually use an opaque carrier pod when interacting with other species. (TOS: "Is There in Truth No Beauty?")

(HH1) A typical Medusan stood 2.3 meters tall, although they could settle to less than a third of that height on their unique, tripedal legs. Their joints were all ball-and-swivel, as with most animals on the planet. Medusans had the ability to splay their legs out on the ground and ride out storms, or pull them in and set up a fast, gamboling trot. Each arm ended in a six-fingered hand, of which three fingers formed dominant graspers and three smaller fingers provided the ability to manipulate objects. Legs end in feet with three toes also capable of grasping objects. The top of a Medusan's body case had three eyes, one over each of the arms, with three olfactory spiracles above the eyes. The mouth was located under one of the three hip joints, the primary excretory apparatus below the hip joint clockwise from the mouth, and the third hip joint protected the cloacal sex organs. (TOS: "Is There in Truth No Beauty?") In the mid-24th century, specialized habitats were incorporated into several Starfleet vessels so that Medusans could serve as onboard navigators through the use of the Interactive Navigational Control System. This would allow the Medusans to operate in symbiosis with the ship's computer in case normal navigational systems were offline. (FASA: Star Trek: The Next Generation Officer's Manual) Timothy Sinclair once touched the mind of a Medusan during his childhood on Titan. He later said the encounter, though disconcerting, was the first time he had ever experienced true peace. (Star Trek: Pendragon) In 2386, three Medusan scientists were part of a research team investigating the ancient technology on Xhe'na'zha. (Star Trek: Pendragon, Star Trek: Sons of Liberty)

Special viewers were required by individuals who sought to see a Medusan outside their protective containers as they filtered out the dangerous frequencies from the eye lids thus lessening the potential damage. (TOS episode: "Is There in Truth No Beauty?") The Medusans were noted as expert starship navigators and had developed interstellar navigation into a fine art which was attributed to their unique sensory system. One theory suggested that the race were capable of electromagnetically identifying as well as locking onto guide stars, even those that were beyond deep sensor range. They were noted as having a distinct form of beauty and were delighted by the many forms of art available to them since their entry into the Federation. (ST reference: The Worlds of the Federation) It was known that their physics and mathematics were among the most advanced of the different cultures within the Federation. (FASA RPG module: The Federation) Medusan starships were known to be equipped with specialist feeding chambers. (TOS novel: Spectre)
