Monthly adoptables (aka. monthlies) are adoptables, mainly Pokéballs, that can be freely adopted for a limited amount of time (usually around a month or so). As Pokéballs--or whatever form they may be--you can adopt as many as you like and are free to level them if you wish just like any other adoptable. They hatch on a certain date, maybe sooner or later than expected, and will reveal the Pokemon (or Digimon, Yu-Gi-Oh! monster, etc.) inside. If you've leveled your adoptable quite well, you might even have a chance of getting a "shiny" version and possibly a medal if yours is one of the top highest leveled. After the monthly adoptables open, they are retired and new ones take their place. The retired monthlies are no longer available unless obtained through trade.
