If you re-installed from scratch from your program CDs, you have to go to the templates directory. Sometimes when reinstalling Photoshop, the preferences will be lost and the programs forgets where the brushes, templates, shapes, etc. Try this, go to Custom Shape tool (the default is line tool and choose custom from there). At the top of the screen, you can see the word "Shape" you can select the presets, however there is a little right arrow in that pop-up menu, if you click on it, it will pull down a menu that says: Text Only - Small Thumbnail - Large Thumbnail, etc.

Hope this helps, I own version 8.0 I know it is old but I hope things haven't changed much since.
