As the name suggests, the map contained two pillbox dugouts, and contained various ramps to get in and out of a dugout. In addition, a large amount of the map was entirely underground in what was presumably an underground base complex. It was composed of the main hill to the west, the cement plains to the east, the lower purgatories to the northeast, far east, and southeast, and the underground hallway. Pillbox Purgatory reappears in Metal Gear Solid: Portable Ops Plus and Metal Gear Solid: Peace Walker, and is the only online map from Subsistence to reappear in Peace Walker's online mode.
