An on monster hit item property is an on-hit property that is intended to simulate special properties of a monster's natural attacks. Consequentially, these are restricted to creature weapons. They are similar in some respects to general on-hit properties, but with fewer options and with duration and DCs based on the possessing creature's hit dice rather than being directly set by the module builder. The amount of level drain and wounding can be set from 1 to 5 for each creature.

On monster hit properties are identified by the effect they inflict upon the target (the creature hit by the creature weapon). They are triggered with every successful hit, at which point the target is allowed a saving throw. The DC of the save for most of these properties is hit dice / 3 + 10. The exceptions are poison and disease, which use the DC of the particular poison or disease, and level drain, which uses an unusual formula in that it depends partly on the target: 10 + attacker hit dice / 2 + <i>target</i> <a href="/mediawiki/Constitution" title="Constitution">constitution</a> <a href="/mediawiki/Ability_modifier" title="Ability modifier">modifier</a>. The type of saving throw is one that is commonly associated with the effect in question. * There is no saving throw for these item properties; instead the usual saving throw for the particular disease or poison applies. The on monster hit: poison property, unlike the general on hit: poison property, uses one of the standard poisons for its effect. The amount of level drain and wounding can be set from 1 to 5 for each creature.
