Richard Brian "Rick" Azari was a Mob Boss who FBI Special Agent Tobias Fornell had spent his entire career trying to put behind bars although Azari had managed to escape justice three times. In an stand-off between himself, Sergeant Jack Kale, Agent Fornell and NCIS Special Agent Leroy Jethro Gibbs, Azari was shot by Kale, resulting in his death straightaway.
