The first time, when he was a young boy of four, he didn't know about the swamp and got trapped in it while helping his mother gather potions ingredients. The second time a heartsick Severus, feeling he had no purpose or reason to continue living, intentionally went to the fen to end his own life, sending a Patronus to his mother.
