They usually do. The bomb used at Hiroshima on Aug 6, 1945 exploded in the air. Whoever launches the nuclear bomb is most likely going to make it blow up in the air because that makes more damage - the blast wave reflects off the ground for extra effect. Also, a contact detonation results in unnecessary nuclear fallout, so it's never used unless he target is a very good bunker.
