If sent by a Shinigami, most go to Soul Society. Otherwise, if the soul has regrets about leaving a certain place, they have a Chain of Fate attached to them and stay in that place and can become Hollow if not sent to Soul Society. If the human committed terrible crimes while alive, they will not go to the Soul Society, but to Hell.
