The Altar of Slad'ran can be found in the Den of Sseratus in Gundrak. This altar becomes available to activate once Slad'ran is killed. When activated the large snake head statue will drop into place on a rotating bridge. This altar is one of three that will cause the bridge to turn allowing the players access to the lass boss Gal'darah.
