The ship's log on the USS Defiant was the method used by the chief medical officer to officially record his final log entry. An entry left by the ship's former medical surgeon was discovered on a monitor at the surgeon's desk by Doctor Leonard McCoy of the USS Enterprise in 2268. McCoy summarized that "According to the ship's log, the medical surgeon down here didn't even know what was going on." (TOS: "The Tholian Web" ) The final draft of the script had actually described this as the medical log, as left by the "ship's surgeon" (versus "medical surgeon").
