Death's coffin was a magical casket made for Death. It's locked 600 ft under the Earth's surface. It was opened when Lucifer unleashed Death. Lucifer, after completing the ritual, digs a ditch, and it rumbles as Death ascends up. When the Horseman's out, Lucifer greets him with, "Oh, hello, Death".
