The Titles and Challenges can be found in the Quests tab by selecting the yellow five-pointed star, similarly like changing from the 'Quests' to 'Achievement Diaries'. At first the tab is absolutely empty. To start off, speak to Hao at the house located north-east of Falador. He is represented by a Five-Pointed Yellow Star on the world map. After you listen to his story, ask about what he is planning to do. He will tell you that he is looking for the inner warrior in you and wishes to test your skills by giving you a set of Challenges. By completing these challenges, he will award you with badges or better known as 'Titles' to prove your effort in accepting and dominating the challenge.
