This is a term that seems to have been deliberately misdefined by Int Mgmt to deny auditors and pcs wins. The tech film showing meter reads shows F/Ns that were computer-generated, not real ones taken from a live person holding the cans. The result? Chaos: processes get overrun, preclears don't "F/N" at exams and so get falsely repaired, and real auditors who call F/Ns correctly can develop W/Hs from it.
