You mean multiplayer maps? The only locked map at any point was Foundation, and it should unlock automatically now if the game is allowed to go online and update itself. Further maps past the default ones have to be downloaded or bought. Campaign levels are unlocked by beating the previous level on your profile, or by beating the level or any subsequent level in coop mode with another profile.
