Toluca Avenue is visited by Travis Grady after he exited the Alchemilla Hospital and decided to check Cedar Grove Sanitarium. Since part of Toluca Avenue is collapsed he enters into the butcher shop to get past the hole in the road. Inside he sees an unknown monster killing a Faceless Nurse and then leaves the store. After leaving the butcher shop, Travis finally reaches his destination. Later, Travis again visits Toluca Avenue after his visit in Sanitarium and to reach Artaud Theater. He also enters again into the butcher shop and sees that killed Nurse has been dragged outside the store.
