You cannot make a poisoned apple. You must either buy them from The Dark Brotherhood, or if you are at a certain point of TDB questline (In the hideout of Lucien Lahnance.), there is a locked barrel with 10 poisoned apples in it. Pick the lock (It doesn't count as a crime.) and you can freely take them. If you come back 3 days later, there should be more apples again.
