In the game, the song plays when Heather Mason speaks with Claudia Wolf inside the chapel. Clockwork Little Happiness also appears twice in the Silent Hill film. It first plays as Rose Da Silva chases her daughter through the streets of Silent Hill, and again when Christabella prays in the church.
