It must be remembered that Nnoitra was a type of Menos Grande prior to becoming an Arrancar. Menos are a conglomerate of many Hollows that feasted upon each other and in doing so become one being. He then underwent further alterations to his appearance when he became an Arrancar. You would have to ask Kubo Tite if there was any change to the hole's location.
