This Book of Shadows existed in an illusionary world created by the Source of All Evil inside the mind of Piper Halliwell. In this world, the sisters were patients in a mental institution and only believed themselves to be witches. When Piper asked Phoebe for the Book of Shadows, she revealed a small notebook concealed behind a drawer. The book included childlike crayon illustrations of demons the Charmed Ones had faced. This further helped Piper realize something was wrong.
