This item can be found inside the Dark Runed Chest after defeating Mal'Ganis in Culling of Stratholme on heroic mode. It was probably made (and maybe owned) by Robert Pierce, owner of Pierce's Pistols in Festival Lane. It is unknown how it ended up in Mal'Ganis's cache.
