The Eyepatch of Clairvoyance was obtainable from an event quest distributed during Season 2. Normally, it is a timed item that lasts only 15 days. However, the picture below shows 90 days because the person had abused the Mission Glitch. After the event had passed, the eyepatch was mainly only seen on GameMasters.
