It asks you to find what's wrong with a picture using a picture of a black and white bedroom, after 21 seconds are up, a picture of a woman with no eyes pops up with the same scream used in Sabotage: Optical Illusions. There was a second version featuring a picture of an old pipe factory, and after 18 seconds, a zombie mask jumps up with a loud roar.
