With Hollows and Arrancar? It's permanent unless somebody or something tears it off. With Visored? That really depends on the person in question, though intense training can slightly increase it. It can average from several seconds to 3 minutes to 15 hours. It does seem that being in Hueco Mundo increases the time by an indeterminate amount.
