This sabotage is by Liquid Generaton, infamous for their screamers. It starts out as a simple test/survey. If you answer all of the questions, it will "calculate" your answers. Once it is done loading, a gif will show a man's head exploding from Scanners with a loud scream. About 4 seconds after, a text will pop up saying You have been sabotaged! Healthy patients will not show any adverse reactions to the tests (apart from a rising heart rate), but arythmia and other conditions will often reveal themselves when the patient is under stress. Stress testing is the act of stressing various parts of software by putting heavy and increased load on inputs (large graphics, more operations, more users, etc.), constrained or reduced resources (low bandwidth, low memory, low storage, etc.), or operating on below recommended hardware (slow CPU, slow hard drive, etc).
