This sabotage is by Liquid Generaton, infamous for their screamers. It starts out as a simple test/survey. If you answer all of the questions, it will "calculate" your answers. Once it is done loading, a gif will show a man's head exploding from Scanners with a loud scream. About 4 seconds after, a text will pop up saying You have been sabotaged!
