It starts in Duke Point, at the ferry terminal there. It immediately heads south as the Duke Point Highway to Cedar, and then heads west to Highway 1, where it merges onto it from the east. The two highways then share a common allignment tnorth just a little ways before Highway 19 diverges west. From the Highway 1 junction, Highway 19 is known as Nanaimo Parkway, and bypasses Nanaimo. Just after passing through Wellington, it officially recieves the Island Highway designation.

The two highways then share a common allignment tnorth just a little ways before Highway 19 diverges west. From the Highway 1 junction, Highway 19 is known as Nanaimo Parkway, and bypasses Nanaimo. Just after passing through Wellington, it officially recieves the Island Highway designation. Highway 19 goes west from Nanaimo, and passes through Nanoose Bay, bypasses Parksville, and then goes northwest. It continues in this direction past Cumberland, Courtenay, and Black Creek to Campbell River. It then goes northwest by Duncan Bay, and continues in this direction to Rooney Lake. Here, it turns southwest. Highway 19 goes southwest to Woss, and then starts going northwest again. It proceeds in this direction through Nimpkish, and Nimpkish Heights to Port McNeill. There are two ferries from Port McNeill. One goes to Cormorant Island and the town of Alert Bay. The other one goes to Malcolm Island and the town of Sointula. Highway 19 does not go on either of these, however. Highway 19 instead goes northwest to its termination in Port Hardy, at the ferry terminal. (En route to Port Hardy, Highway 19 passes through a junction with Highway 30).
